The posterior band of the inferior glenohumeral ligament is a thick and long fibrous structure that is attached to the infero-superior aspect of the glenoid and underlies the supraspinatus tendon, and the acromion. It is thought to provide medial stabilization for the shoulder joint [22]. The IGHL measures approximately 13.5-17.5 mm in length. The distance from the inferior edge of the glenoid to the superior edge of the acromion varies from 13 to 18 mm, and the distance from the superior edge of the acromion to the superior border of the bicipital groove ranges from 35 to 53 mm [22]. In cases of larger AI, the distance from the inferior glenoid margin to the anterior acromion margin is increased, and the distance from the anterior acromion margin to the inferior edge of the deltoid muscle increases. In addition, the length of the posterior band of the inferior glenohumeral ligament is increased. In long-standing cases of AI, the anteroposterior position of the acromion is displaced to the posterior-inferior portion. Therefore, the IGHL is stretched by the anterior and inferior displacement of the acromion. However, as the acromion is rotated medially, the posterior band of the inferior glenohumeral ligament is pulled forward and posteriorly.
